Complex Care Practitioner (Learning Disabilities and Autism)

An exciting opportunity has arisen for an enthusiastic and experienced Complex Care Practitioner (Learning Disabilities and Autism) to join the Continuing Healthcare (CHC) Team within North West London.
This post is ideal for a Registered Learning Disabilities Nurse (RNLD) or suitably experienced Allied Health Professional who is passionate about improving outcomes for people with learning disabilities and complex health needs.
Working within a specialist multidisciplinary team, you will assess, monitor and review individuals in line with the National Framework for NHS Continuing Healthcare. You will work closely with service users, families, health and social care professionals, provider organisations and local authorities to ensure individuals receive person-centred, safe and effective care that meets their assessed needs.
The role offers an excellent opportunity to develop expertise in Continuing Healthcare, commissioning and complex care assessment while gaining experience across a range of health and social care services. Main duties of the job
- Effectively provide quality and commissioning case input and oversight to clients with a learning disability and complex care needs.
- Plan, implement and evaluate care plans for clients and their carers using evidence based and person-centred principles
- Provide specialist nursing advice and support to clients with a learning disability and complex care needs, their families, or other providers, clinicians or professionals involved in the care, using evidence based practice
- Liaise and work in a joined-up way with the client, their family, relevant health and care professionals, and all other health and social care agencies involved in the care of the person
- Contribute to, or lead on (as appropriate), relevant assessments and reviews that support care planning for complex clients with a learning disability
- Represent the ICB in complex care panels to provide clinical oversight, risk management, and help inform discussions effecting resource allocation, including making recommendations to commissioners on what care should be commissioned to support that individual to meet their needs.
- Cultivate knowledge of local employment, community and universal services that individuals can access to promote their health and wellbeing, as well as knowledge of highly specialized and/ or bespoke interventions
- Help quality assure and monitor community and/or inpatient providers the ICB commissions for individuals with a learning disability with complex care need

Experience
Essential
- Full understanding of Section status relating to mental health patients (S2/S3/S37/41/S117) and the impact on service delivery and funding streams.
- Excellent working knowledge of the national NHS Learning Disability and Autism (previously Transforming Care) programme and agenda
- Knowledge of the CPA/ CTR process
Desirable
- Knowledge of the Community Care Act
- Knowledge of the Continuing Healthcare Framework (2018) and its application.
- Awareness of NHS funded care and how its application is in practice
Skills and knowledge
Essential
- Ability to assess and plan individual care needs in partnership with patients/ their carers and MDTs
- Ability to work in partnership and liaise with a range of health and care professionals involved in our client’s care
- Ability to case manage patients/individuals with complex needs
- Ability to make decisions autonomously, based on evidence
- Ability to communicate and adapt communication style to suit a range of audiences e.g. clients, carers, commissioners, care staff etc.
- Ability to present assessments justifying outcomes
Desirable
- Leadership and management skills and ability to facilitate, supervise and assess others in the delivery of care
- Ability to give clear and concise reports against performance targets
- Confidence to challenge professionals and decision makers based on evidence
Education/Qualification
Essential
- 1st level Registered Nurse with current registration with the NMC.
- Registered Nurse with Learning Disabilities (RNLD) with significant post-registration experience in a senior clinical role.
- Evidence of CPD relevant to role
- Educated to degree Level
Desirable
- Degree in health related topic
- Education at Masters Level
